I'm not expert on growing edible mushrooms by any means, but it looks as if you didn't case the spawn?

If not, a mixture of peat and vermiculite, wetted and heated in the microwave for eight minites is said to make a good casing material.

It's cased. The mycelium broke through the top all nice and even.

P.S. add a teaspoon of lime (not the fruit!) to your verm/peat mix to increase pH and decrease likelihood of contamination.

Oyster shell works well too: you can get it at a feed store, as it's used as an additive for chicken feed. I used to keep chickens and $10.00 worth could probalby case the world!
